Online Registration Form

The form will open in a new window when you click the link at the bottom of this page. It contains 6 sections that will allow you to sign up for the cruise (Costa Crociere ) + for the seminar (ChiroSuisse):

• Section 1: Put the name(s) + address of all the people sharing the same cabin (4 people max per cabin and per registration form). Please fill in the names and all the additional info regarding all the passengers sharing your cabin, do not just fill in for you. The cruise company needs to have all the information and not only the information regarding you (as a seminar participant).
• Section 2: Choose your cabin type (you can click on the room type to choose what you want and click on the price list in order to view the price list)
• Section 3: Choose the type of transportation you will use (click on the city name in order to determine the city of departure) (please pick only one choice)
• Section 4: Information regarding the seminar
• Section 5: Billing information
• Section 6: Insurance information (if you need to make a comment regarding your registration there is a place for comments). Please don't forget to check that you have read and do accept the general conditions and to certify that your passport is still valid 6 months after the cruise! Swiss holders of an ID card are allowed to the cruise but a valid passport is still to be recommended. Families with 3 small children may share one cabin but this is subject to approval. In such a case just add the third child's name under comments; on approval you will be contacted by Stohler Agency and the fee for the third child will be an additional CHF 160.
